  • Clear description of the nature of your organisation, including its key functions and whether it is for-profit or not-for-profit;
  • Statement of the exact amount requested or support in kind (please include summary of previous years support if applicable);
  • Summary of supported activity (-ies);
  • Breakdown of amount if it is to cover numerous costs;
  • Date of the event or activity.
